Autism Program Pre-Treatment Guide
Welcome to the Miami Stem Cell Autism Program. Our goal isn’t just to deliver stem cells – it’s to maximize outcomes.
Moreover, we take a whole-body approach, not just a neurological lens. Specifically, before infusion, we tailor a pre-treatment plan to clear roadblocks nutrient deficits, gut dysbiosis, autoimmunity, mitochondrial stress. As a result, your child’s body is primed to receive the stem cells, calm inflammation, and repair as effectively as possible.
To prepare your child’s body to receive the stem cells, we recommend doing the following tests 4–8 weeks before your procedure:
- Gut Health & Dysbiosis Test (Analutos GI panel)
- Folate Receptor Antibody Test (FRAT) – *medical prescription needed to order the test

Gut Health & Dysbiosis Testing (Analutos)
- Type: Urine test that provides comprehensive gut panel (e.g., GI-MAP or GI Effects)
- Purpose: Full picture of gut status — permeability, microbiome balance, yeast/parasites, calprotectin (inflammation), and zonulin (leaky-gut). Also flags toxic metabolites that can aggravate behavior and neuroinflammation.
- Why it matters: 🧠 The gut–brain axis is pivotal in ASD. Dysbiosis + inflammation can worsen behaviors and immune activation and can limit stem cell efficacy.
- Timing: 🗓️ Complete before treatment so we can address overgrowths, replenish beneficial species, and repair the gut lining in advance.

Transparency on FMT: We’ve attached information on fecal microbiota transplant (FMT)—the treatment we recommend when a child tests positive for severe dysbiosis. If your child’s panel indicates that FMT may help, then we’ll discuss the benefits, risks, timing, and costs before any decision is made.
Folate Receptor Antibody Testing (FRAT)
- Type: Blood
- Purpose: Detects folate receptor autoantibodies that can block folate transport into the brain (Cerebral Folate Deficiency).


Why Does It Matter?
- These antibodies block folate from entering brain cells, disrupting normal folate metabolism and neurological function.
- Folate is essential for language, cognition, attention, and overall brain health.
- Folinic Acid (medication called Leucovorin) uses a different transport pathway to enter cells, that is converted to folate and allows folate metabolism to resume normally — often improving speech, social interaction, and cognition in FRAT-positive children.
- Optimizing folate pathways can set the stage for a better neurodevelopmental response to stem cells.
Timing:
🗓️ Ideal: 4–8 weeks before stem cell therapy (allows dose titration if needed).
🩸 If blood draws are hard: We understand how is to draw blood in children with autism, but don’t worry, we can do the test on the procedure day. In that case, we will start Leucovorin weeks prior to procedure (when appropriate) and draw the FRAT on procedure day since we will already have IV access.
How to order the FRAT test online
Prescription needed: if you would like to proceed with the test, we’ll give you a prescription to order the FRAT test online. Also, if appropriate, we will prescribe your child Leucovorin.
If testing before the procedure:
Take the kit to a local lab for the blood draw.

Bring the unopened kit on procedure day; we’ll collect the sample via IV access.
We recommend starting Leucovorin prior to treatment (when appropriate).
We’ll handle shipping; please email results once you receive them.

What Does the FMT Protocol for ASD Look Like?
Pre-Treatment Plan
- Gut Preparation: 2 weeks of antibiotics (with antifungals/antiparasitics if indicated), ending 48 hours before FMT.
- Bowel Cleanse: Performed the day before FMT.
- Liquid Diet: Optional, done the day before and on the first day of FMT.
Treatment Phases
- Loading Dose:
- Administered 1–2 days after bowel cleanse.
- Provides a bolus of FMT to rapidly establish the new gut microbiome.
- Maintenance Dose:
- Smaller, extended doses over weeks/months.
- Helps reinforce microbiota engraftment, sustain long-term benefits, and support immune, inflammatory, and gut-brain/gut-liver balance.

- Medical Management Fee: $2,500 (one-time, non-refundable; applies to non-stem cell patients).
- This fee applies only for patients who are not having Stem Cell Treatment with Dr. Weiss’s.

FMT FAQs:
FMT is the infusion of healthy donor stool material into the gut to restore microbial diversity, rebalance the microbiome, and improve health.
Methods include capsules, oral powders, colonoscopy, enemas, and tubes. Capsules/powders are increasingly preferred for safety and convenience.
FMT has proven up to 90% effective in treating recurrent C. difficile infections and is being studied in ASD and other conditions.
Most side effects are mild and short-lived (fever, nausea, bloating). The key to safety is rigorous donor screening and high-quality lab preparation.
